Belize Boosts Malaria Detection Tenfold as It Strengthens Post-Elimination Surveillance

BELMOPAN — Belize has achieved one of the most significant surveillance gains in Latin America and the Caribbean, increasing the proportion of suspected malaria cases receiving a diagnostic test from 2.9 percent in 2019 to 23.6 percent in 2025. This sharp rise in testing capacity is reinforcing the country’s ability to rapidly identify and contain potential imported cases as it works to safeguard its malaria-free status.

After earning malaria-free certification in 2023, Belize has entered a new phase focused on protecting that achievement through faster, smarter, and more resilient surveillance. Recent independent verification of targets under the Regional Malaria Elimination Initiative in Mesoamerica and the Dominican Republic (RMEI), administrated by the Inter-American Development Bank (IDB) and financed by The Gates Foundation, The Carlos Slim Foundation and The Global Fund, shows concrete improvements across detection, treatment, reporting, and vector control. These gains have reinforced the country’s capacity to prevent the return of malaria.  

Beyond expanded testing, Belize has made major strides in laboratory performance. The percentage of laboratory reporting that meets quality standards increased nearly fourfold, rising from 16.7% to 62.5%, enhancing the reliability of surveillance data.

Belize has also demonstrated strong clinical readiness. All confirmed malaria cases received appropriate first-line treatment within 24 hours of diagnosis and all received treatment according to national guidelines. This rapid response capacity is critical to preventing isolated cases from becoming new outbreaks.

Prevention efforts at the community level have expanded as well. Coverage of vector control interventions among at-risk populations increased from 28.4% to 69.4%, largely driven by expanded indoor residual spraying. This broader protection reduces vulnerability in receptive areas and strengthens the country’s defensive perimeter against reintroduction.

These results reflect deliberate investments in operational readiness. Belize has reinforced passive case detection in health facilities, strengthened border surveillance in high-mobility zones, expanded rapid diagnostic testing at community level, and trained rapid-response teams to investigate suspected cases swiftly. A fully operational situation room now supports real-time malaria analysis, enabling data-driven decision-making across districts.

The IDB, through the RMEI, continues to support Belize through results-based financing, technical assistance, and innovation. This model links measurable improvements in surveillance and response capacity to performance incentives.

In a region marked by high population mobility and environmental pressures, preventing malaria’s return depends on resilient systems rather than isolated achievements. Belize has maintained its zero cases status, its certification as a free malaria country.  Maintaining elimination is not a static milestone, but an ongoing effort — built on faster detection, stronger reporting, expanded protection, and continuous preparedness.
