Integrated Health Services Strengthening Program

The general objective is to reduce avoidable non-communicable diseases (NCD) morbidity and premature mortality in Trinidad and Tobago (T&T) by strengthening an integrated, patient-centered health system. The specific objectives are: (i) Strengthen continuity, interoperability, and data-driven health service delivery for NCD prevention and management; (ii) Improve continuity of care for NCD prevention, early detection, and management; and (iii) Strengthen sustained adoption and routine use of integrated health services, prevention programs, and digitally enabled care models to improve patient engagement, preventive behaviors, and disease management.
