Care integration for fragile older adults
The general objective of the Technical Cooperation (TC) is to contribute to improve the quality of care for dependent older adults, as well as the medical management of the situation of frailty and multimorbidity often associated with dependency. The specific objective is to disseminate knowledge on the care of dependent older adults through a web platform that integrates a knowledge base, training modules and software to support clinical decision making. There is no free and open access to a clinical decision support system (CDSS) in the region. Given the shortage of geriatric experts in the region for specialized consultations, this software has the potential to be disruptive, expanding the possibility of problem solving from the first level of care.
