You can choose the provider of your preference. To receive dental care, you will be required to pay for the services received and then file a claim for reimbursement.

You can submit a claim for services you paid out of pocket through the Cigna Health Benefits website, mobile app, fax or by postal mail. Click here for the claim form if you submit a claim by fax or postal mail. Claims cannot be filed via email.
The fastest and preferred method is to submit your claim on the Cigna Health Benefits website or mobile app. For specific steps to file a claim, click here. Once Cigna processes your claim(s), you will receive a Settlement Note (SN) containing details about your claim, including a summary of the services claimed and the amount paid by the Medical Program. It will also indicate if additional information is required.
