How to Submit a Claim

For prescriptions filled inside the U.S. and Puerto Rico: Nearly all pharmacies in the U.S. and Puerto Rico are in-network and your claim will be adjudicated at the pharmacy. However, if you fill your prescription at an out-of-network pharmacy, you must pay out-of-pocket and then file a claim with Express Scripts for reimbursement. PLEASE NOTE: If you use an out-of-network pharmacy, you will only be reimbursed for 50% of the amount you paid for the prescription. Use the Express Scripts website (expressscripts.com) to submit a claim online. You can also call the number on your Express Scripts ID card for assistance with filing a claim.

For prescriptions filled outside the U.S. and Puerto Rico: You must pay our-of-pocket and submit your claim to Cigna Healthcare.

Omada for Diabetes and High Blood Pressure Help

Omada® is an online service that provides members with a dedicated virtual health team and a personalized health plan to manage chronic conditions like diabetes, lower blood pressure, and improve overall health—all at no cost to you. The program includes:

  • A welcome kit with smart devices to help monitor progress and show results. The devices are yours to keep!
  • Support from a certified coach and specialist who offers personalized guidance about nutrition, exercise and self-care
  • Access to online peer communities to share successes

Only members needing specialized care for diabetes or hypertension based on their prescription drug claim history are eligible for Omada.

About the List of Allowed Drugs (Formulary) in the U.S. and Puerto Rico

As of July 1, 2024, we updated our pharmacy drug list or “formulary” for prescription purchases in the U.S. and Puerto Rico. Our plan now covers medications only listed in this formulary. If a brand medication is not included, there will be an alternative brand name drug within the same therapeutic class that is covered.

How can you review what drugs are on the formulary?

There are three ways you can find out what drugs are on the formulary and, if a drug is not on the formulary, what alternative drugs are in the same therapeutic class are on the formulary:

How can my doctor request that a non-formulary drug can be covered for me?

There are a couple of ways that your physician can contact Express Scripts’ Prior Authorization team to request a review.  

Typically the provider will be asked to document if the preferred alternative(s) has been tried and if the patient has any allergies to the preferred alternative(s). Generally, the Express Scripts Prior Authorization team has a turn-around time of 48 hours on non-urgent reviews and 24 hour turn-around time on urgent reviews. Notification of approval or denial is sent both to the member and the requesting physician.

How to Submit a Claim

For prescriptions filled outside the U.S. and Puerto Rico: When you purchase a prescription drug outside the US and Puerto Rico, you will pay out-of-pocket for it, and submit a claim for reimbursement with Cigna Global. Claims must include a copy of the valid prescription and the receipt for your purchase with each refill. Outside the US and Puerto Rico, all prescriptions are retail and require a $5 co-pay for up to 30-day supply regardless of the pharmacy you choose. 

  1. Log in to the Cigna Health Benefits website (www.cignahealthbenefits.com) 
  2. Click “Claims” in the menu bar 
  3. Click the “Submit a claim” button 
  4. Select the member who received care in the drop-down menu and identify whether that member is covered by other insurance and whether the claim is related to an accident 
  5. Click the “Next Step” button 
  6. Add the invoice date, amount, type of care, and country in which care was given 
  7. Click the “Next Step” button 
  8. Use the “Choose files” button to add your claim documents (make sure you also add any supporting documents, such as prescription, proof of payment, etc., if required) 
  9. When you are finished adding documents, click “Save this invoice” 
  10. On the next screen, review the summary of your claim information to ensure it is correct and then click “Submit claim”

For prescriptions filled inside the U.S. and Puerto Rico: Use your Express Scripts ID card to fill any U.S. prescription at an in-network pharmacy. Most of the major retail pharmacy chains are in-network with Express Scripts. At these pharmacies, your claim will be adjudicated at the pharmacy and you will pay only any applicable co-pay. If you fill your prescription at an out-of-network pharmacy you must pay out-of-pocket and then file a claim for reimbursement with Express Scripts. U.S. Home Delivery of Maintenance Medications

Express Scripts Pharmacy has a U.S. home delivery pharmacy program available within the United States, Puerto Rico, and U.S. Virgin Islands, for maintenance drug prescriptions delivered ONLY to a U.S., Puerto Rico or U.S. Virgin Islands address. Express Scripts cannot send mail-order prescriptions to overseas addresses. 


To order a new home delivery prescription, you can make a request by mail or your doctor can request electronically.  

  •  If you want to make a request by mail: Request a prescription from your provider for a 90-day supply with refills 
  1. Download the order form 
  2. Include your delivery address 
  3. Update health conditions and allergies information 
  4. Include payment information (credit card or US checking account) 
  5. Include your prescription  
  6. Mail the completed order form and prescription to: 

          EXPRESS SCRIPTS PHARMACY 
          PO BOX 66301 
          ST LOUIS, MO 63166-6301 

  • Electronic request:  Ask your doctor’s office to send your prescription electronically to Express Scripts Home Delivery, NCPDP 2623735, or via fax to reduce your waiting time for prescriptions. Have your doctor call 1-888-327-9791 for faxing instructions, if needed. If your doctor’s office handles the request electronically you do not need to mail anything to Express Scripts. 

To manage your existing home delivery prescriptions, go to Express Scripts website.
