Due to incompatibility with printer drivers some of the graphics, text and margins will be shifted when you open up this MS Word File. The only solution to this problem is by using the PDF format.
Only English version currently available at the moment.

Installation Instructions for Adobe Acrobat Reader
After the download is complete: (If you did not open up the file directly from it's location).
- Double-click the newly downloaded file (ar40eng.exe).
- Follow the instructions on your screen.
If there is a failure at any point during the installation of Acrobat Reader, the installer performs a complete uninstall. For this reason, it is important not to close the installer application by clicking its close box in the upper right corner of the background window after clicking the "Thank You" dialog box that appears at the end of the installation. If you wait for a second or two, the installer will automatically close the background windows after the installation is complete.

Once you open up a PDF File writing your browser you will see the following buttons: You will mainly use the 3 buttons described below in order to completely fill out the document.
The Hand Button is used to place the cursor and left click in the section you intend to fill information with, note that only after you select this button you will be able to fill out the document.
The Magnifying Glass Button is used to increase your view of the document for easier reading. If you select this button you must then select the hand button to replace the cursor on the document.
The Print Button is used for printing the document: (If you are filling out the IADB Personal History Form you will only need to print out pages 1 to 4). You may of course print out the pages 5 to 6 before filling out the form, because you will need these pages to complete section 5 (Experience Classification) of the Personal History Form.
Please note that you will not be able to save the document with the information you have filled out; make sure you have completely and accurately filled out the document. Otherwise, you will have to start all over again. You can use the Tab key on your keyboard to move between fields on the document only after you have placed the cursor on the document with the Hand Button.
